In the last blog, I discussed about the basic information related to the Disability Certificate for Person with Disabilities. In today’s blog, I present to you, step by step process for obtaining the Disability Certificate.
An online application is to be submitted for obtaining a Disability Certificate and ID card, whether fresh or renewing the existing one.
Steps to obtain a Disability Certificate online
Visit the website https://www.swavlambancard.gov.in/
Click on the ‘Register’ button & create an account on the UDID web portal
Fill in the required details viz personal details, disability details, employment details (if applicable) & identity details
Upload the required documents and submit
After registration, you will get login credentials (enrolment no./UDID no.) on your registered mobile number and email id
Login using the enrolment no./UDID no. & date of birth and click on ‘Apply online for Disability Certificate’
Upload the required documents and submit the data to the CMO Office/medical authority
This data is verified by the CMO office/medical authority and a specialist(s) is assigned for assessment
The specialist doctor assesses the disability of the candidates and accordingly give his/her opinion
The case is then reviewed by the medical board and a disability percentage is then assigned to the PwD
The disability certificate and UDID is generated by the CMO Office and dispatched to the disabled persons
